We have been authorised by the following councils to carry out medicals for their taxi drivers:

What I will need to bring

Below is a list of items you will need to bring to the doctor for your driver medical assessment:

Taxi Medical form (our doctors have spare copies of forms, except for the Sefton council form which must be obtained from the council)
Driving licence (or passport if your licence is currently with DVLA or lost)
Your driving glasses and optical prescription (contact lenses must be removed for the ‘uncorrected vision’ eye test);
A list of all your current medication, the dosages and the dates you began taking them
Any hospital letters or details of hospital consultants who are treating you;
You may be asked to provide a urine sample to test for diabetes
Proof of address (please see list above)
Medical summary (Refer to list above to see if this applicable to you)

How to Get a C1 License for Minibus or Vans?

Eligibility: Be at least 18 and hold a full UK driving license (Category B).
Apply: Complete the D2 form and get a D4 medical form completed by a doctor.
Theory Test: Pass the Driver CPC Part 1 (Theory Test).
Practical Test: Pass the Driver CPC Part 3 (Practical Test).
CPC Training: Required for professional drivers.
