What to bring to the appointment
D4 Medical form/Taxi form/ Employment medical form (our doctors have spares if required)
- Driving license/ ID Card
- Proof of address – e.g. phone or utility bill (Taxi medical only)
- Glasses if worn for driving
- Details of medical conditions
- Details of any medical consultants you are seeing
- Details of regular medications taken
For the following taxi councils you will need to bring your medical summary:
Burnley Council, Manchester Council, Oldham Council, Stockport Council, Salford Council and Walsall Council
