Any doctor who holds full registration with the GMC (General Medical Council) and is licensed to practise medicine in the UK can complete the DVLA D4 Driver Medical. We have a panel of 60+ doctors who are annually trained in Group 2 medical assessments. 

It’s important to note that not all GPs offer this service, and some may not have immediate appointments available, which could delay the process. Therefore, using a service like Motor Medicals that specializes in driver medicals can be more efficient, ensuring the examination is thorough and in line with the latest DVLA requirements.

All our doctors carry copies of the D4 medical form, however we always advise bringing your own copy to your appointment just to be on the safe side, you can get a copy of the the D4 medical form here:

For Taxi Medical forms, some clinics may carry council forms however this is not always the case. To stay prepared for your appointment please find links to each Taxi medical forms below.

 

Whilst the doctor completes the D4 medical form for you, you will need to complete the D2 form yourself. For more information on how to complete your D2 or D47P form please visit the DVLA website.

The doctor will examine you in accordance with the requirements of the relevant D4 Medical Form.

During the medical our Doctor will:

  • Complete an eye test (if you wear glasses/ contact lenses, one test without and one test whilst you’re wearing them)
  • Record your blood pressure
  • List on the D4 Form any medication you’re currently taking
  • Ask about your previous medical history (these questions are on the form and are compulsory).
  • You may be asked to provide a urine sample to test for diabetes

 

Once the medical is completed you sign the back page to confirm you have given the correct information to the Doctor and you must then check you are happy the form is fully completed and correct before ticking at the end of the form. This is very important as if there are mistakes or omissions the form is returned to you by DVLA who deem this the driver’s responsibility.
Once you have posted the form to DVLA they make a decision on fitness, they will either accept your application or reject it and explain why.

Approximately 15 minutes but if your medical history is complicated expect it to take longer.

Yes, all our medicals include an eye test for no extra charges.

 You will be asked to complete the test with and without your glasses/ contact lenses. Please bring a container to place your lenses in if you wear contact lenses.

Below is a list of the six items you will need to bring to the doctor for your driver medical assessment:

1) Driving license or Passport

2) Bank statement or utility bill for proof of address

3) A list of any medications that you take (if applicable)

4) Your glasses or contact lenses (if worn)

5) Any Required forms either D4 or Taxi council forms and Medical summary

6) Outstanding payment in CASH

You may be asked to provide a urine sample to test for diabetes

 

Of course, you will receive an email with a VAT receipt alongside your appointment confirmation email. 

The DVLA D4 medical form is valid for 4 months from the date of the doctor’s signature (ie date of D4 form completion), so you must send the paperwork in to DVLA within 4 months of your medical for it to be accepted, for group 2 licence categories to be added to your licence or renewed. Please note this does not mean you must pass your HGV/PCV theory or practical driving test within 4 months.

If this is your first application, when you receive your licence back from DVLA with the additional provisional categories these are available to view online and will not show on your physical photocard driving licence.

You will for Taxi Medicals: Currently, all councils require a medical summary to be seen by the doctor. Please consult the medical form and ensure the form can be completed by any registered doctor before booking.

We can not accept the NHS as per council rules. 

HGV Medical: A medical summary is not required for a HGV (D4) medical.

The medical summary we require for all taxi medical assessments can be acquired by simply asking the receptionist at your GP office to print out your medical summary. This process is free and typically can be done same day while others may require you to come back and collect it after a while. 

Please ensure you have the medical summary before your taxi medical appointment and we strongly recommend only booking your appointment once you have the medical summary as some GPs have wait time for printing.

In the event that you wish to cancel your medical appointment, you must
provide us with at least 24hrs notice (Monday to Friday). Where you provide us with
this notice, we will refund the fee paid by you for the cancelled medical appointment

Any appointments cancelled within 24hrs will be charged a cancellation fee.

This policy helps us maintain efficient operations and keeps our services affordable for all customers. Should you need to reschedule, we will do our best to accommodate your request and find a new time that suits your schedule.

All taxi driver medical assessments are completed by a GMC registered doctors to a Group 2 standard.

Please note- You must bring your medical summary to your Taxi driver medical. 

A taxi driver medical examination typically includes the following:

1. Medical history review from your medical summary/medical records
2. Physical Examination inc height and weight measurements, Blood pressure, heart rate, respiratory rate
3. Examination of specific body systems: Such as cardiovascular, respiratory, neurological, musculoskeletal, and sensory systems.
4. Vision Test including visual acutiy and peripheral vision test
5. Urine Test (if required by the council)
6. ID review inc ID check and proof of address

After completing the medical check, the doctor will review the results and fill out the necessary DVLA forms, such as the D4 for HGV drivers. If all the standards are met, the medical practitioner will sign off on the check, confirming your fitness to drive.

You will then need to submit these documents to the DVLA, along with any additional information they require for your license application or renewal. The process can vary depending on whether you’re a new driver or looking to renew your existing license. If there are any health concerns that arise during the medical check, the doctor may request further tests or provide guidance on managing any conditions that could affect your driving.

 

The first step is to understand why you did not pass. The doctor will provide details on any health concerns that need addressing. Depending on the issue, you may be able to take steps to improve your health and reapply for the medical check, such as by managing a medical condition with medication or lifestyle changes.

In some cases, you might need to undergo further tests or receive treatment. It’s important to follow the doctor’s advice and any treatment plans carefully. Once your health has improved to the required standards, you can usually retake the medical check.

 

Certain conditions can prevent you from passing the medical required for HGV and taxi drivers. For instance, severe heart conditions, uncontrolled diabetes with a history of severe hypoglycemia, epilepsy, and certain neurological disorders might disqualify you. Poor vision that doesn’t meet the DVLA’s minimum standards is also a common reason for not passing the medical examination.

Substance abuse, including alcohol and drugs, can lead to a failed medical check, as these can significantly impair your ability to drive safely. Mental health conditions that are not well-managed and could affect driving performance are also taken into consideration.

However these are quite subjective and dependent of individual circumstance which is why it’s important for our Doctors to fully review your medical history.
