How to Access Your Medical Records

Some councils require a recent medical summary or full records before your taxi medical can be completed. Use one of the options below.

    Request Records from Your GP
    1. Contact your GP surgery by phone, email or through its online portal.
    2. Ask for the type of records required by your licensing council.
    3. Check that the records are recent enough for your council.
    4. Bring a printed or digital copy to your appointment.
Allow enough time. GP surgeries may take several days to prepare medical records.
    Use the NHS App
    1. Sign in to the NHS App and complete identity verification.
    2. Open your GP health record and check what information is visible.
    3. Ask your GP surgery for full access if the required details are missing.
    4. Confirm your council accepts NHS App records before relying on them.
Important: not every council accepts NHS App access instead of printed medical records.

Requirements vary between councils. Use the council checker before booking to confirm whether you need a medical summary, full records or an appointment with your registered GP.

    Taxi Medical FAQs

Find answers about taxi driver medicals, private hire medical requirements, council forms, medical records, eyesight tests and booking a taxi medical near you.

    What is a taxi medical?

A taxi medical is a health assessment required by many local licensing authorities before someone can obtain or renew a taxi, hackney carriage or private hire driver licence.

During a taxi driver medical, the doctor reviews your medical history, checks your blood pressure, eyesight and general health, and completes the medical form supplied by your council. Taxi medical requirements vary between councils, so drivers should check their local authority’s rules before booking.

How much does a taxi medical cost?

Our taxi and private hire medical appointments cost £53, with the eyesight test included. The appointment price covers the medical assessment and completion of the relevant council taxi medical form, provided you bring the correct documents and records.

Some councils may require additional medical records from your GP, and your GP surgery may charge separately for preparing those records.

What happens during a taxi driver medical?

A taxi driver medical normally includes a review of your medical history, current medication and any health conditions that may affect safe driving. The doctor may also carry out:

    • A blood-pressure check
    • An eyesight test
    • A height, weight and BMI assessment
    • A general physical health assessment
    • Completion of your council medical form

The exact assessment depends on the medical standards used by your local taxi licensing authority.

Do I need medical records for my taxi medical?

Some councils require a recent medical summary, while others require full medical records. A small number of licensing authorities may require the taxi medical to be completed by your own registered GP.

Use the council checker before booking to confirm which taxi medical records you need. Arriving without the correct records may mean the doctor cannot complete your private hire or taxi medical form.

Can I use the NHS App for my taxi medical?

Some councils accept medical information displayed through the NHS App, but others require a printed medical summary or full records directly from your GP surgery.

Before relying on the NHS App for a taxi medical, check that your council accepts it and make sure the required medical history is visible. Basic NHS App access may not show enough information for every council taxi medical assessment.

Where can I get my council taxi medical form?

Your taxi medical form is normally available from your local council’s licensing website. You should download the correct form for your taxi, private hire or hackney carriage licence and bring it to your appointment.

You can also visit our taxi medical forms page to find commonly requested council medical forms.

What should I bring to my taxi medical appointment?

To help your taxi medical appointment run smoothly, bring:

    • Valid photo identification
    • Your council taxi medical form
    • Your medical summary or full records, if required
    • Your glasses or contact lenses, if worn
    • A list of your current medication

Check your council requirements before attending because the documents required for a private hire medical can differ between licensing authorities.

Is an eyesight test included in the taxi medical?

Yes. An eyesight test is included in our taxi medical appointment price. The doctor will check whether your vision meets the relevant driving standard and record the result on your council taxi medical form.

Bring your glasses or contact lenses if you use them for driving. Some council forms may also request information from an optician, depending on your eyesight history or the wording of the form.

How long does a taxi medical take?

A straightforward taxi medical appointment is usually completed during one clinic visit. The time required can vary depending on your medical history, the council form and whether the doctor needs to review detailed medical records.

Bringing your completed council form, photo ID and the correct medical records helps avoid delays.

Can any doctor complete a taxi medical?

This depends on the licensing authority. Many councils allow a GMC-registered doctor to complete the taxi medical, while some insist that the form is completed by your own GP or by a doctor with access to specific medical records.

Check your council’s taxi medical requirements before booking to make sure the appointment will be accepted.

What if I have high blood pressure, diabetes or another medical condition?

Having a medical condition does not automatically mean you will fail a taxi medical. The doctor will consider the condition, your treatment, how well it is controlled and whether it may affect your ability to drive safely.

Bring accurate medical records and a current medication list. In some cases, further information from your GP or specialist may be needed before the council taxi medical form can be completed.

Can I book a taxi medical near me?

Motor Medicals provides taxi medical appointments at 40+ UK clinic locations. Use the clinic locator to search by postcode, town or current location and find a convenient taxi medical clinic near you.

Once you have checked your council requirements, you can book a taxi or private hire medical online at your preferred clinic.

Will I receive my completed taxi medical form on the day?

Where the doctor has all the required information and there are no outstanding medical concerns, the council taxi medical form can usually be completed during the appointment.

Completion may be delayed if records are missing, additional medical evidence is required or your council has specific requirements that cannot be confirmed during the appointment.

What happens if my council is not listed?

If your licensing authority is not shown in the council checker, speak to our team before booking. We can review the council taxi medical form and advise whether one of our GMC-registered doctors can complete it.

Some councils require the medical to be carried out by your registered GP, so it is important to confirm this before making an appointment.
