BPA Skydiving Medical Forms

Tandem Skydiving Medical Form

For Tandem skydiving you will first need to complete your self medical declaration form 115A found on BPA site and only complete  the Doctors medical certificate 115B below if you have any health concerns listed in 115 A

 

Solo Skydiving Medical Forms

For Solo skydiving you will first need to complete your self declaration of fitness form 115C found on BPA site as well as the Doctors medical certificate 115D below.

What does the Skydiving Medical include?

The specific tests involved in a skydiving medical assessment can vary depending on the requirements of the skydiving centre and depending on solo or tandem skydiving. The following are commonly included in your skydiving medical:

** Solo Skydiving medical only

What will I need to bring?

1. Driving license or Passport

2.Medical summary from GP

3. A list of any medications that you take (if applicable)

4. Your glasses or contact lenses (if worn)

5. Previous medical certificates

When do I need a Skydiving Medical?

Solo 

Lisence Application 

When applying for skydiving licenses (e.g. BPA in the UK), a medical certificate may be required

Aged 40+

Any one over the age of 40 must get a medical before they jump. 

Tandem 

Aged 70+

Any one over the age of 70 must get a medical before they jump.

All Skydivers 

You will also need a medical before a jump if you suffer with any of these conditions:

Epilepsy

Severe head injury

Recurrent blackouts

Any kind of brain or nervous system disease

Hypertension

Heart or lung disease

Diabetes

Mental illness

Drug or alcohol addiction
