You will need to fill in any grey colours areas including:
- Right side of the first page
- The bottom of every page – Your name and DOB
- Left side of the back page
The Doctor will fill in the rest (green sections)

What To Expect During Your HGV/D4 Medical
D4 Medical Eye Test
An eyesight evaluation is a crucial part of your HGV/D4 medical. During this evaluation, the examiner will check both your distance vision and your peripheral vision.
