  • Glasses if worn for driving
  • Details of medical conditions
  • Details of regular medications taken

TAXI ONLY

  • Proof of address - e.g. phone or utility bill (Taxi medical only)
  • Medical Records/ Summary
  • Medical Summary / Full Records

Taxi / Private Hire Drivers on Diabetes Medication will need 4 weeks of Blood glucose readings 2x a day with no low sugar.
We can not see any drivers on insulin.

Please note- At least a medical summary is now required for taxi medicals, for all councils. Some require Full Mediacl Records. This can be obtained from your GP practise reception.


We may be able to accept the NHS app depending on the council.
